Our work is rooted in compassion, dignity, and service to the community. Position Summary The Social Worker will work closely with the SECOR Cares guests to assess needs and develop service plans to navigate resources and improve the overall well-being of guests. Applicants for this position should have a depth of knowledge in resource navigation, particularly in the areas of housing, food, and health. Key Responsibilities Conduct comprehensive and personalized assessments to understand guest needs and barriers Develop, implement, and review individualized service plans regularly maintain detailed, confidential records of guest interaction in compliance with legal and organizational standards Track, and document individual barriers to report consistent needs in our growing community and develop recommendations on best practices to address them Connect guests with resources including housing, healthcare, financial literacy, tech education, and employment services. Liaise with other service providers while advocating for guests and ensuring continuity of care. Develop a calendar of planned health education activities for the benefit of guests' overall well-being

Licenses, Registrations, and Certificates:

Bachelor's or master's degree in social work or related field Licensed Social Worker (LSW, LMSW, or equivalent preferred) Minimum Qualifications Excellent communication, advocacy and organizational skills Possess a strong understanding of local community resources and ability to navigate social service systems Minimum two years of experience as a social worker or in healthcare
