Role:
As a Social Worker with Arden House , you will have the opportunity to make a meaningful impact on the lives of patients/residents and their families/significant others. As a member of our interprofessional team, you will provide psychosocial services, support, and guidance to our patients/residents, helping them achieve the highest practical level of physical, mental, and psychosocial well-being.
Responsibilities:
Administrative:
Assist with planning and implementing a comprehensive Social Services program, ensuring compliance with federal and state regulations.
Advocacy:
Work with patients/residents, families, and significant others to promote and protect their rights, interests, and well-being.
Clinical:
Conduct comprehensive psychosocial assessments, identify social, emotional, and psychological needs, and develop person-centered care plans.
Education:
Educate patients/residents, families/significant others, and staff on the role of Social Services, health care decision making, advance directives, and available community resources.
Discharge Planning:
Identify discharge goals, develop discharge plans, and coordinate with patients/residents, families, and staff to ensure a smooth transition.
Additional Responsibilities:
Provide therapeutic interventions, facilitate groups, and support patients/residents and families/significant others in coping with transitions and adjustments.
Requirements:
Masters degree in Social Work or Human Services field required Must possess any certifications/licensures as required by State of employment to practice in long-term care 1-3 years of supervised social work experience in healthcare setting working directly with individuals preferred Additional certification such as Geriatric Case Management, Hospice & Palliative Care, Gerontology, Clinical Social Work, Health Care, Nephrology, Mental Health, and/or Substance Use preferred How to
