Licensed Social Worker Job Requirements:
Experience as a LTC social worker within a nursing home. Knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities CT Social Work License (Required)
Admission:
1 year (Preferred) Long term care: 2 years (Preferred)
Licensed Social Worker Responsibilities:
Plan, organize, development and implement all required social services to meet the needs of residents in accordance with federal, state and local guidelines that govern skilled nursing facilities. Performs all documentation requirements for social service documentation in a timely fashion. Completes all admission assessments and evaluations Works with the interdisciplinary team to create a plan of care for residents. Works with and provides support to residents, families and staff. Attends weekly Care Plan and Medicare meetings. Receives and attempts to resolve resident grievances. Coordinates resident discharge planning with all relevant departments and assists with scheduling outside services. Participates in monthly team meetings such as QA and Safety.
