A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Shift:
Shift 1, Days Position Summary To provide direct services as a part of a multidisciplinary team the master's prepared clinician will provide direct care including individual (particularly children/adolescents), couple, family and group therapeutic services to patients, including assessments, treatment plans, crisis intervention and discharge planning.
Essential Duties & Responsibilities Direct Patient Contact:
Provide individual and family therapy to children and their families Participates in direct care to assess needs and coordinate services Maintains daily/weekly scheduled appointments in electronic records systems Provide therapeutic services to patients and their families Work in collaboration with multidisciplinary team members including external partners Completes required documentation and paperwork including biopsychosocial assessments and treatment planning and implementation in a timely manner Participate in our high risk program Demonstrates cultural competency and commitment to patients including LGBTQ+ community Maintain case load dependent on need Participates in clinical supervision with LCSW Provides on-call support on a annual rotational basis if required by program Provide crisis management, support and triage Adheres to all Core Values (Compassion, Pursue Excellence, Cooperation and Collaboration, Upholds Honesty, and Supports Innovation). Adheres to all Absolutes (Privacy and Confidentiality, Professional Appearance, and Responsibility and Commitment). Minimum Qualifications Master's degree in social work, counseling, marriage and family therapy Possession of valid clinical license in Connecticut (LMSW, PC-A, MFT-A) Valid driver's license a must for in home services Comfort with using DSM 5 and understanding diagnoses Must have excellent writing and communication skills, be highly organized Must be able to work as a part of a team
Preferred Qualifications:
LCSW, LPC, LMFT
