The Social Worker will support residents and their families by addressing psychosocial needs, facilitating care planning, and ensuring a smooth transition throughout the continuum of care. This role plays a key part in enhancing residents' quality of life and maintaining compliance with regulatory standards.
Key Responsibilities:
Conduct comprehensive psychosocial assessments for residents upon admission and as needed Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team Provide counseling and emotional support to residents and families Assist with discharge planning, including coordination of community resources and services Advocate for residents' rights and ensure dignity and respect in all interactions Facilitate communication between residents, families, and healthcare providers Maintain accurate and timely documentation in compliance with state and federal regulations Participate in care plan meetings and interdisciplinary team discussions Address issues related to adjustment, grief, behavioral concerns, and long-term care placement Stay current on healthcare regulations and best practices in long-term care social work
Qualifications:
Bachelor's or Master's degree in Social Work (BSW or MSW) required Previous experience in a skilled nursing facility or healthcare setting preferred Strong interpersonal, communication, and organizational skills Ability to handle sensitive situations with professionalism and empathy Knowledge of Medicare, Medicaid, and long-term care regulations is a plus
