Job Description
Wolcott Hall Nursing Center is a 57-bed skilled nursing facility located right off of Route 8 in Torrington, CT. Wolcott Hall maintains an excellent community reputation and a very family-like atmosphere. Apple Rehab is a family owned and operated company that treats residents and staff like family too. Our expert team of senior management is located at our home office, right in Avon, CT, ensuring superior care from a local company. Our leadership is not across the country, but rather in your backyard. A Per Diem Social Worker fulfills the vital role of Social Work Director to those in need. We are looking for a Per Diem Social Worker to assist in coverage for our current Director's maternity leave. This position uniquely follows residents by supporting them through their individual continuum of care beginning at admission and throughout their transition.
Social Service Functions:
The Director of Social Services is responsible for implementing and documenting a plan of care related to the resident's social/emotional needs in the electronic medical record. The Director of Social Services is also responsible for collaborating with the nursing, MDS and rehabilitation departments to facilitate discharge plans for residents. The Director of Social Services is responsible for informing and educating the staff/resident/family/responsible party of the resident's personal and property rights, as well as his/her rights related to health services as pertaining to the resident. The Director of Social Services maintains updated resources for residents and families in regards to community resources and services, as well as service agencies and organizations. Maintains a record of all reported concerns, assists in resolving them and assures appropriate follow up and documentation. The Director of Social Services is easily accessible to residents, family and staff. Provides timely referrals to appropriate agencies for assistance to the resident/family (i.e. conservatorship application, Medicaid application, etc).
Qualifications:
Bachelors or Masters Degree from an accredited program in Social Work or related field. Previous experience in a long term care facility as well as with current Requirements of Participation regulations is preferred. Must be knowledgeable of admissions and discharge procedures in conjunction with all current laws, as well as be familiar with residents' rights and documentation requirements in accordance with current regulations and/or guidelines. Attention to detail, good follow through skills and ability to prioritize multiple tasks. Must be innovative and able to find creative ways to serve the variety of resident needs and maintain the customer focus. Must have a vehicle and a valid drivers' license. Demonstrates cooperation in assisting others and ability to work as a team. Patience, tact, enthusiasm and a positive attitude toward the residents and their support networks. Ability to remain calm in stressful situations. Ability to communicate with residents, staff and family members in a professional manner.
