The Opportunity We are currently seeking a dedicated Licensed Social Worker What You Will Do Assist in the planning, organization, implementation, and evaluation of the facility's Social Services Department Ensure social services are delivered in compliance with federal, state, and local guidelines and in alignment with Harborview's values and policies Help develop and monitor individualized care plans for residents, addressing their social and emotional needs Provide ongoing support to residents and families during admissions, transitions, and care conferences Recommend appropriate equipment, services, or referrals for outpatient and home health support Participate in policy development and recommend improvements to strengthen care delivery Organize and participate in in-service training sessions for the social services team and other staff Stay up-to-date on regulatory changes and professional standards related to long-term care social work Advocate for residents and serve as a liaison among staff, families, and external agencies Collaborate with interdisciplinary care teams to ensure holistic, person-centered care What You Must Have Bachelor's degree in Social Work (BSW), or in a human services field such as sociology, psychology, special education, or rehabilitation counseling Minimum of 1 year supervised social work experience in a healthcare setting working directly with individuals Knowledge of federal and state long-term care regulations Strong interpersonal, communication, and problem-solving skills A resident-first mindset with a deep commitment to dignity, empathy, and respect Ability to manage sensitive situations with professionalism and compassion Must meet all applicable licensure and certification requirements in the state of practice Benefits - Things we want you to know We offer a comprehensive benefits package, including: 401(k) managed by Merrill Lynch DailyPay Health Insurance with Anthem Blue Cross or Cigna Dental, Life, Vision, and Disability Insurance Paid Time Off Continuing Education Credits Career advancement opportunities!
