Job Description
Lee Health is seeking a Brief Intervention Social Worker to provide short-term, evidence-based support to patients and families across our Southwest Florida health system. In this role, you will conduct rapid psychosocial assessments, deliver focused counseling and crisis intervention, and connect patients with community resources to address mental health, substance use, and social needs. You will collaborate closely with interdisciplinary care teams, support safe transitions of care, and contribute to quality and population health initiatives in a mission-driven, patient-centered environment.
