Job Description:
Schedule:
Full Time, 40 hours per week
Shift :
10 hour days (4 Shifts per week)
Locations:
Main Location is Orlando but covering our IPUs in Altamonte Springs and Kissimmee when needed. Interacts with facility/hospital case managers to coordinate hospice services. Evaluates the psychosocial needs of patients and their families. Develops and implements psychosocial care plans based on patient and family needs. Conducts initial assessments at patient residences. Assists patients and families in obtaining financial resources. Provides services as determined by the interdisciplinary team and reports to the registered nurse. Communicates with caregivers and makes referrals to community resources. Educates and assists patients and families in completing Advance Directives. Offers individual or group counseling to patients and families. Assesses and documents family bereavement needs from admission through the hospice process. Maintains professional documentation in the electronic medical record. Other duties as assigned.
Knowledge, Skills, and Abilities:
Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements. Knowledge of electronic medical record. Ability to follow standard precautions and uses personal protective equipment when indicated. Ability to complete hospice Orientation and Training, as necessary. Knowledge of current counseling theories and practices in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee. Knowledge of local, state, and federal entitlement programs. Knowledge of advanced directives. Knowledge of home health and hospice care. Basic computer skills and proper telephone etiquette. Flexible and willing to make changes, i.e., schedule adjustments or change in assignment. Demonstrate excellent interpersonal skills. Ability to work without close supervision. Computer proficiency, including experience with Microsoft Office programs (Word, Excel, Outlook, and PowerPoint). Verbal and written skills necessary to effectively communicate with various members of the health care team, other health facilities, community health-related organizations, various external parties, and regulatory agencies. Ability to use office equipment such as telephone, personal computer, copier, fax machine, etc. Ability to support departmental and organizational mission.
Education:
Master's [Required]
Field of Study:
in social work, psychology, counseling or related field with applicable state licensure
Work Experience:
2+ experience in the social work field is required [Required] Healthcare/hospice experience [Preferred]
