Job Description

Looking for a change? Consider Care Management! We are an established full-service Care Management company who assists older adults and individuals with disabilities. Our company is growing and we are looking for a dynamic Social Worker, or related social service field to support seniors and their families in The Villages and surrounding area. We cover Lake, Sumter, Marion and Citrus counties. The ideal candidate will have availability during the Monday-Friday workweek and enjoy the flexibility of managing their own schedule while providing compassionate support to seniors and their families. Case Management experience a plus Must live within 20 miles of The Villages Must have community experience and knowledge of community resources Marketing experience is a plus Flexibility is a must Weekend on call once a month is required Travel in Central Florida is required Must have reliable transportation This is a part time position including travel Ability to work 10-15 hours/week Pay will be discussed upon interview Responsibilities include: Conducting Assessments Developing/implementing individualizes care plans Care coordination and resource support Monitoring progress by conducting regular visits Accompanying clients to medical appointments Educating and supporting clients and their families Documentation and maintaining records Communication with families and providers Advocacy and guidance for seniors
Qualifications:
Bachelor's or Master's degree in Social Work or a related human services field Strong communication and organizational skills Proficient with basic technology, including email Microsoft Word and electronic documentation Compassionate, reliable, and able to work independently Join us in making a meaningful difference in the lives of older adults.
