Contract Length:

Weeks Shift:

8-Hour Night Shifts (40 hours/week) Responsibilities Assess patients' psychosocial needs and develop appropriate care plans. Coordinate discharge planning and community resource referrals. Collaborate with physicians, nurses, case managers, and other healthcare professionals. Provide crisis intervention, counseling, and emotional support to patients and families. Maintain accurate documentation in accordance with hospital policies and regulatory requirements. Advocate for patients to ensure continuity of care and positive outcomes. Requirements Active Social Worker license (as required by the State of Georgia). Hospital or acute care experience preferred. Excellent communication, assessment, and documentation skills. Ability to work effectively in a fast-paced hospital environment.
