Responsibilities Conduct comprehensive patient assessments to evaluate psychosocial needs, cultural considerations, and family dynamics. Provide individual, family, and group counseling sessions utilizing clinical counseling techniques such as motivational interviewing and cognitive behavioral therapy. Coordinate community resources to support patients' social, emotional, and practical needs, including housing, financial assistance, and spiritual support. Assist with discharge planning by collaborating with healthcare teams to ensure seamless transitions between inpatient, outpatient, home care, or assisted living settings. Maintain accurate medical documentation in compliance with HIPAA regulations and social work practice standards. Participate in interdisciplinary team meetings to develop personalized care plans aligned with hospice & palliative medicine principles. Offer crisis intervention when urgent psychosocial issues arise, ensuring immediate support for patients and families. Experience Proven experience working with geriatric patients in hospice or palliative care environments. Strong background in child & family counseling, mental health counseling, or behavioral health services. Familiarity with medical terminology, ICD coding (ICD-9/10), CPT coding, and medical records management. Experience conducting diagnostic evaluations and providing psychotherapy or behavioral therapy in clinical settings. Knowledge of community resource coordination within diverse cultural backgrounds and familiarity with Medicare regulations. Hands-on experience with crisis management, discharge planning, and case management across inpatient or home & community care settings. Demonstrated ability to work collaboratively within an interprofessional healthcare team while adhering to clinical ethical guidelines and HIPAA standards.
