Overview:
POSITION SUMMARY
The Case Manager (CM) II works in collaboration with the patient/family, physicians and interdisciplinary team members to ensure patient progression through the continuum of care and to independently develop a plan of care for each assigned patient from admission through discharge. Identifies, initiates and manages optimal patient flow/throughput to enhance continuity of care, smooth and safe transitions, patient satisfaction, patient safety, and length of stay management. Utilizes their knowledge and expertise to provide clinical oversight and direction for Case Manager I Mission Partners and to evaluate the individual patient's needs via comprehensive assessment and managing care coordination and discharge planning. Assesses and responds to patient/family needs by coordinating efforts of the interdisciplinary team members and identifies and resolves barriers that hinder effective patient progression of care.
Qualifications:
REQUIRED QUALIFICATIONS
Education:
Bachelor's degree in of science in nursing (BSN) with 2 years healthcare setting experience OR Master's degree in social work (MSW) or Master's degree in Professional Counseling with 2 years of healthcare or child welfare experience OR Bachelor's degree in Social Work (BSW) at least 2 years of healthcare or child welfare experience with a master's degree in leadership or healthcare related field
Licensure/ Certification:
Registered Nurse License OR Social Worker License (LSW) OR Licensed Bachelor' Social Worker (LBSW) OR Licensed Professional Counselor (LPC) in IL or MI American Case Management Association (ACMA) CM certification - test taken within six months of hire and passed within one year of hire.
Other Skills/ Knowledge:
Solid computer skills, including proficiency with Microsoft software. Completion of the OSF Ministry Preceptor course within 6 months of hire. Demonstrates a high level of emotional intelligence. Excellent interpersonal and verbal and written communication and ability to interact with diverse populations. Critical and analytical thinking skills. Ability to multitask and prioritize in a fast-paced environment. Strong knowledge of discharge planning, utilization management, case management, performance improvement, and managed care reimbursement. Strong understanding of pre-acute and post-acute levels of care and community resources. Ability to work independently and exercise sound judgment in interactions with physicians, payors, patients and their families.
PREFERRED QUALIFICATIONS
Education:
Master's degree in science nursing (MSN) or
MSW Experience:
1 year of inpatient case management experience
Licensure/ Certification:
