POSITION SUMMARY
The Social Worker shares the responsibility of identifying social and emotional needs of each participant. The position assists with arranging services to meet the needs of the participants, providing liaison between the participant, family, facility staff, and the community. Assess all participants behavioral health needs, gathers and analyzes information from available sources including family, agencies, friends and others. The Social Worker formulates and communicates treatment information to other members of the interdisciplinary team (IDT) and collaborates with the IDT on ongoing treatment and care decisions. The role also coordinates or provides services including individual, group, family, marital and/or group psychotherapy and serves as a resource to staff and community regarding psychosocial issues. REQUIRED QUALIFICATIONS
Education:
Master's Degree in Social Work from school approved by the Council on
Social Work Education Experience:
A minimum of one years' experience working with the frail or elderly population or, if the individual has less than one year of experience but meets all other requirements must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
Licensure/ Certification :
Current licensure as a Licensed Social Worker (LSW) or a Licensed Clinical Social worker (LCSW). Current American Heart Association CPR Certification.
Other Skills/ Knowledge:
Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem solving skills, with the ability to be detail oriented.
PREFERRED QUALIFICATIONS
Experience:
2 years of hospital, counselling, community facing, or senior focused, social work or relevant fieldwork placement in mental health field or home care Experience working as a member of a an inter-disciplinary team in a hospital, nursing home, or community-based setting is preferable.
