Assist the physician and other team members in understanding the significant social and emotional factors related to the patient's health problems, perform evaluations, and participate in the development of the plan of care for the patient Assess psychosocial situation of patient/caregivers and help them understand and follow the medical recommendations made by the care team Provide information and referral services for patients and families regarding practical and environmental needs, including financial assistance programs and resources, and serve as the liaison between patients/families and community agencies Assist the patient and family with personal and environmental difficulties which pre-dispose toward illness or interfere with obtaining maximum benefits from medical care Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care. Position Qualifications:
Candidate(s) must have a Master's Degree from a Social Work accredited by the Council on Social Work Education One (1) year of Social Work experience in a health care setting, hospice experience preferable. Must be able to drive to patient's current residency (assisted living facility, house, etc.) and have a valid driver's license and auto insurance
