Full Job Description Job Summary:
The Palliative Oncology Social Worker II, in outpatient palliative care, assesses patient/family needs and coping skills that support patient's unique cultural and spiritual beliefs. Provides holistic patient/family centered care across the treatment continuum. Provides education, clinical interventions, and community resources specific to the patient/family need. Serves as member of the patient's multidisciplinary care team and conducts advance care planning conversations. Coordinates with the care team and other coworkers to facilitate the patient's goals and high‐quality care, focusing on the patient as the center of services
