The Social Worker assesses patient/family needs and coping skills that support the patient's unique cultural and spiritual beliefs. Provides holistic patient/family centered care. Provides education, interventions and community resources specific to the patient/family need. Provides crisis intervention, complex or long‐term placement linkage to appropriate community resources. Participates in discharge planning and treatment planning. Coordinates with the care team and other coworkers to facilitate the patient's goals and achievement of quality and cost‐effective outcomes.
