In this role, you won't just complete tasks—you'll help shape quality of life, protect resident rights, and ensure every individual is seen, heard, and supported. This position is located at Deaconess Midtown or Gateway campus.

Position Requirements:

BSW Case Manager Graduate of a program accredited by the Council on Social Work Education with a Bachelor of Social Work (BSW). Recommended to continue education with obtaining Masters in Social Work (MSW) - Deaconess will assist in continued education through tuition reimbursement program. Experience in health care, crisis intervention, counseling and/or social agency preferred. This can include internships or other employment.
