Neighborhood Liaison-R2

Neighborhood Liaison-Family Services Geminus Corporation partners with community-based health care providers, social service organizations and early childhood education programs to help increase access to high-quality social health care for children and families across Northern Indiana. Geminus Corporation is a member of the Regional Care Group.

Position Purpose:

To provide case management services to children and families in the Division of Child and Family Services through the Community Partners program. 1. Provide home based services 2. Develop safety and risk assessment 3. Facilitate child and family team meetings 4. Develop service plans 5. Provide documentation through computer programming 6. Complete client surveys to assess cases

Minimum Hiring Requirements Education:

Bachelor's degree in social work, psychology, sociology, or a directly related human service field from an accredited college. Other bachelor's degrees will be accepted in combination with a minimum of three years' experience working directly with families in a human service field. Associate degrees in a related human services field with a minimum of three years' experience working directly with families in a human service field. High school diploma or equivalent with a minimum of five years' experience working directly with families in a human service field.

Experience:

Minimum of 2 years' experience working with youth and families providing home based services providing child welfare services.

Location:

This position works out of our Valparaiso office and covers counties in R2 which include; LaPorte, Porter, Benton, Starke, Newton, Pulaski and Jasper. Pay Range $21.04/
