Assess client needs and work with agencies and institutions ensure clients get the help they need. Monitor client and work with other service providers as necessary to address client needs. Provide limited counseling to client and their family.

  • Bachelors in Social Work (or other related field)

  • Employee assistance program Responsibilities of Family Care Coordinator /
Case Manager:
  • Establish initial contact with families within 24 hours of referral to assess safety,
  • Initiate safety planning, and connect families with resources to meet immediate needs;
  • Conduct team meetings and maintaining communication with the family and providers
  • Facilitating the implementation of each family's Action Plan
  • Make frequent and consistent contact with DCBS personnel
  • Provide direct services and education targeted at reducing risks, improving child well-being, and preserving child placement with the family per program guidelines.
