Job Description

Job Summary Join our dedicated team as a Social Service Worker, where you will play a vital role in supporting individuals and families through compassionate, client-centered services. The Social Service Worker provides direct services to residents and their families, St. Mary's staff, social agencies working with St. Mary's residents, and community groups. Participates in providing in-service training to St. Mary's staff regarding issues of advocacy, human rights abuse/neglect, and services of the Social Services Department. Helps ensure the protection of rights for St. Mary's residents and their families. Provides support to residents and their families to maximize the physical, mental, social and emotional development of all residents served. Those services shall also strive to enhance the coping ability of families and protect the safeguards of their human and civil rights. Essential Duties (This list may not include all of the duties assigned) Responsible for professional and ethical delivery of social services to the resident, the resident's family/guardian/agency having custody and ensuring that high standards of professional practice are maintained in the delivery of Social Services. Responsible for serving as a liaison to families, or agencies seeking placement at St. Mary's, effectively communicating the pre-admission process documentation necessary for admission, scheduling pre-determination meetings, and communicating decisions of the pre-determination committee. Responsible for planning, coordinating and carry out the admissions process with the resident, family, or agency. Responsible for representing the Social Service Department through the orientation process, by educating new hires on the policies related to recognizing and reporting abuse/neglect, consequences of failure to report, legal ramifications of abuse/neglect, of the infirmed. Assess residents' psychosocial needs to develop and implement individualized support plans. Coordinate community partnerships and activities that enhance resident engagement and well-being. Investigate allegations of abuse or neglect and ensure timely reporting to appropriate agencies, including DCFS and EPS. Conduct facility tours and provide information to prospective residents, families, and referral agencies Follow all safety policies and perform additional duties assigned by leadership. Requirements Valid Driver's License Bachelor's Degree required; Human Services related field preferred Criminal background check
Pay:
$17.00 - $19.00 per hour
