Job description: Social Worker (Part-time/PRN)

Overview:

Audubon Hospice of Baton Rouge is seeking a compassionate and dedicated Social Worker to join our team. The Social Worker will assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, neglect or abuse and plans intervention based on evaluation findings.

Responsibilities:

Conduct comprehensive assessments of clients' needs, strengths, and goals. Develop individualized care plans in collaboration with clients and their support systems. Provide counseling and crisis intervention services to individuals and families. Coordinate with external agencies and service providers to ensure clients receive necessary support. Advocate for clients' rights and access to resources Monitor clients' progress and adjust care plans as needed. Maintain accurate and up-to-date documentation of client interactions and services provided. Educates patients and families o, and assists in, preparation of advanced of advanced directives. Other duties are delegated by the Clinical Supervisor.

Requirements:

Bachelor's or Master's Degree in Social Work (LMSW/LCSW) Strong crisis management skills and ability to manage high stress situations. Proficiency in conducting assessments and developing care plans.
