Job Description

Social Worker LTC Join our dynamic team at Oak Knoll Rehabilitation and Healthcare Center as a Social Worker! As a vital member of our Social Services Department, you will play a key role in enhancing the lives of our residents by providing medically-related social services, completing comprehensive assessments, and developing individualized care plans. Responsibilities:
Assist in overseeing the development, implementation, supervision, and evaluation of the Social Services Department Complete comprehensive assessments for assigned residents and develop individualized care plans Identify and promote individualized, non-pharmacological approaches to care that meet the mental and psychosocial needs of each resident Ensure residents have access to appropriate treatment and resources Treat all residents with dignity and respect, promoting and protecting their rights Establish a culture of compliance by adhering to all facility policies and procedures Maintain confidentiality of protected health information and adhere to all facility policies and procedures Perform administrative requirements, including completing assigned reports and submitting them to the Social Services Director Contribute to the facility's efforts to maintain and improve quality of care through participation in quality improvement initiatives
Requirements and Qualifications:
Bachelor's degree in social work or a related field One year of supervised social work experience in a healthcare setting Ability to read, write, speak, and understand the English language Ability to make independent decisions and deal tactfully with personnel, residents, family members, visitors, and government agencies Must be supportive of the team, contribute to, and be an example of teamwork Basic computer skills Ability to move intermittently throughout the workday and perform other common physical movements as needed for the position
Working Conditions:
