Job Description
Social Worker-Oncology Holyoke Medical Center is an independent community hospital that has served the healthcare needs of the Pioneer Valley for over 125 years. We continue to grow and address the needs of our community through quality and patient centered care. The specialized services and programs offered by Holyoke Medical Center s Oncology Department provide our patients with the assurance they want and deserve, delivering compassionate cancer treatment and care that is nationally recognized for quality. Our Oncology team is seeking a Social Worker to evaluate and respond to the needs of individuals and/or their families. They provide care based on the documented clinical assessment of the patient's needs, which includes a culturally sensitive assessment of biophysical, psychosocial, environmental educational, and discharge planning factors. Will also develop clinical formulation that is based on assessment data and grounded in clinically sound theory that directs treatment toward individualized, solution-focused interventions.
