Matter:
Conduct comprehensive psychosocial assessments and develop individualized care plans to address residents' emotional, social, and discharge planning needs. Provide counseling, intervention, and ongoing support to residents and their families while advocating for resident rights and quality of life. Collaborate with an interdisciplinary care team to coordinate services, facilitate care conferences, and ensure resident-centered care. Connect residents and families with community resources and post-discharge support services to promote successful care transitions. Qualifications forSuccess:
Bachelor or Master of Social Work (BSW or MSW) Current, unencumbered MA Social Worker license (LSW) Prior experience in a long-term care setting strongly preferred Ability to communicate fluently in English- both verbally and written Benefits for
