We're hiring a Care Manager / Social Worker to provide psychosocial assessments, care management, and caregiver support in accordance with the Massachusetts Adult Foster Care (AFC) Program in the Lowell, Lynn, and Boston areas. If you're a social worker who enjoys building relationships, coordinating services, and making a day-to-day difference in clients' lives, this role is for you. What you'll do Client assessment & care management Complete and collect AFC-required assessments, data, and program forms Conduct initial and ongoing psychosocial assessments to determine appropriateness for AFC Maintain accurate client charts and documentation from admission onward Support the client's Plan of Care by addressing social, emotional, and environmental needs Home visits & qualified setting compliance Conduct initial and ongoing home setting evaluations to ensure AFC-qualified setting requirements are met Complete the Qualified Setting Assessment Tool accurately and on time Identify risks and collaborate with the team on solutions that keep members safe Caregiver support (in collaboration with the RN) Help select, evaluate, train, and support AFC Caregivers alongside the Registered Nurse Provide ongoing guidance to strengthen caregiver skills and member outcomes Community resources & advocacy Connect members and families with community supports as appropriate (MassHealth/Medicaid, Medicare, SSI/SSA, SNAP, housing resources, etc.) Coordinate with providers, facilities, and community agencies to reduce barriers to care Required qualifications Bachelor's degree in Social Work or related field Licensed Social Worker in Massachusetts (LSW) or Bachelor's degree plus 2+ years of recent clinical experience with older adults and/or adults with disabilities 2+ years of experience working with elderly and/or disabled populations (recent experience preferred) Strong documentation, communication, and interpersonal skills Knowledge of Massachusetts community resources and benefits systems Valid driver's license, reliable insured vehicle, and ability to travel locally Comfortable using computers and electronic documentation systems Preferred qualifications Experience in AFC/GAFC, Adult Day Health (ADH), or other long-term services and supports (LTSS) Care coordination, case management, or home-based social work experience Benefits (Full-Time) We offer competitive compensation based on experience.
