Health Imperatives (HI) is a multi-service organization dedicated to reducing health and wealth disparities while advancing meaningful opportunities and economic security for people south of Boston. Our mission is to improve the health and well-being of low-income or vulnerable families and individuals in Southeastern Massachusetts. The communities we serve, including Brockton, New Bedford, Plymouth, the South Shore, South Coast, Cape Cod, Martha's Vineyard, and Nantucket, have some of the highest poverty rates and largest wealth gaps in Massachusetts. For nearly 50 years, we have addressed the complex needs of people who are disadvantaged by poverty and discrimination and have fallen through the cracks of mainstream systems of care. $28.10 per hour plus benefits

DUTIES :
Under the supervision of the Site Supervisor, the Home Visitor's responsibilities include the following: Provide home visiting/ parent education services to eligible teen parents. Participate in group support services. Serve as a role model and give support and encouragement to parents and their families to help raise healthy children. Track, record and maintain participant data as required. Participate in all site visits. Participate in community outreach and help/improve family's access services in their communities. Disseminate appropriate materials in strategic places throughout the community detailing the need for early prenatal care, parenting education and prevention of child abuse. Participate in on-the-job training in infant care, nutrition, parenting skills, childbirth education and child development as required by funders and Health Imperatives. Other duties as assigned by the Director/Supervisor.
REQUIRED QUALIFICATIONS
High school diploma and three to five years of early childhood experience or bachelor's degree in social work, early childhood education, human services or related field and one to three years of early childhood experience. Travel required. Reliable transportation, valid MA driver's license, auto insurance, registration, and a safe driving record required. Knowledge of community resources helpful. Excellent written and spoken English language skills. Must maintain a flexible schedule.
