• $65,000 annually (based on experience) Job Overview Lorien Bel Air is seeking a compassionate and motivated Social Worker to support the psychosocial needs of seniors residing in our Long-Term Services (LTS) and Skilled Nursing Facility (SNF) .

This role focuses on advocacy, case management, care coordination, and interdisciplinary collaboration in a long-term care setting. Licensed and non-licensed social work candidates are encouraged to apply. Non-licensed candidates, new Social Work Graduates must be willing and able to obtain social work licensure within the first year of employment . Key Responsibilities Assess, evaluate, and monitor residents' psychosocial and emotional well-being Advocate for seniors and their families to support dignity, autonomy, and quality of life Provide case management for short-term rehabilitation and long-term care residents Collaborate daily with nursing, therapy, physicians, and interdisciplinary teams Assist with admissions, discharge planning, and transitions of care Participate in and coordinate care plan meetings with residents and support systems Maintain ongoing communication with residents, families, and external agencies Assist with advance directives, MOLST forms, and capacity documentation Ensure accurate, timely, and compliant documentation per state and federal regulations Track required certifications and ensure regulatory compliance Qualifications Licensed Social Worker (LSW/LCSW) preferred Non-licensed candidates will be considered with expectation of licensure within 12 months Bachelor's or Master's degree in Social Work (BSW or MSW) Minimum 1 year of healthcare, long-term care, or case management experience preferred Strong interest in healthcare or clinical social work Excellent written and verbal communication skills Strong organizational, critical thinking, and interpersonal skills Ability to thrive in a team-oriented, fast-paced healthcare environment Benefits & Compensation 80 hours vacation + 40 hours sick time (roll-over eligible) 6 Personal Days 7 Paid Major Holidays CareFirst Medical Insurance Tuition Assistance (supports licensure and professional growth) Annual Cost of Living Increases 401(k) with employer matching Employee Assistance Program (EAP)
