Reporting to the Transitional Age Youth Transition Coordinator, the Transitional Age Youth (TAY) Coordinator is responsible for providing case management services in collaboration with the Transition Coordinator and other cross-functional teams. The TAY Case Coordinator will focus on building a partnership with young persons to create a solid foundation for the transition into adulthood through intensive case management services; housing stabilization; job coaching and access to supported employment opportunities; resource procurement and advocacy. The TAY Case Coordinator is responsible for engaging participants in immediate needs assessment and procurement of resources as a part of a service delivery team. The positions work within the limitations of the organizations resources and in compliance with grants, contracts, and agency policies and procedures.
