A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Job Description

Primary Job Duties :
Responsible for critical decision-making. Must regularly exercise discretion and judgment to perform work, which is intellectual and varied in character, the accomplishment of which cannot be standardized as to time. Assists with coordination of Independent Living admissions/transfers/discharges with other departments. Identifies psycho-physical and psycho-social needs of the Independent Living residents. Ensures residents are in the appropriate levels of care (Independent, Assisted, Memory Care, Respite, Skilled Nursing and Rehab) within the community and are receiving the supportive services needed to obtain optimal levels of health. Collaborates with other members of the community team in identifying and recommending additional services or transitions within the continuum of care for residents with changing needs. Experience Required 2-3 yrs exp working with older adults/seniors Education Required Masters or better in Social Work Licenses & Certifications Required Lic.
