We arelooking foran experienced,licensedmedicalsocial worker(LMSW)withpsychosocialassessment and advocacy skills. TheSocial Workerwillprovidepsychosocial assessment, screening, counseling, and crisis intervention services on a consultation basis to patients and families,utilizinga variety of treatment modalities including individual, family, group, and specialized counseling. Works collaboratively with the multidisciplinary team todeterminethe need for services; affect positive patient and family outcomes; and, toassistin securing community resources to promote a continuum of care. Primary Duties:
Advocates for the patient and family. Maintains availability to the patient/family as aresource tofacilitatecommunicationamong providers andmonitorservicesrendered.

Assesses andassistspatients in referral and application for funding sources/eligibility forentitlement programs.

Assistspatients in the exercise of their rights while encouraging autonomy and participationin their care.

Assistspatients and families in adjustingto illness, coping with lifestyle changes precipitated by illness or injury, and addressing issues of a terminal illness. Conductstimelypsychosocial assessment interviews withpatients and families toidentifyproblems requiring social workinterventions andplans forappropriate services.

Explores the need for guardianship and other legal proceedings.

Providescounselingrelated to the environmental needs,family/interpersonalconflict (such as domestic violenceand abuse), or physical and mental illness,using a variety of treatment modalities.

Usesclinical skills in a crisis to resolve conflicts. Maintains strict standards for patient confidentiality and patient-related information in addition to the security ofelectronic information.

Complies withdepartment, hospital, Peer Review Organization (PRO), andMaryland statepolicies on confidentiality.

Completestimelyandappropriate documentation. Participates in departmental monitoring,statistical reporting requirements,and facility-wide performance improvementefforts. Attendsin-service and training programstomaintainand expand clinical skills and knowledge, serves as an interdisciplinary team member on committees,andparticipatesin inpatient, staff, facility, and community education programs including supervision of students as assigned. Provides consultation to other disciplines when psychosocial aspects of care or coordination with intensive community resources are necessary to achieve desired treatment outcomes.

Works collaboratively andmaintainsactive communication with members of the multidisciplinary care teamto effecttimely,appropriate patientmanagement through the continuum of care.
Qualifications:
Master's degree in social workfrom a school accredited by the Council on Social Work. 1-2 yearsSocial Workexperiencein a hospital, inpatient, psychiatric, or medical setting/specialtyarea. Licensed Medical Social Worker(LMSW)in the State of Maryland. This position has a hiring range of USD $60,632.00 - USD $107,494.00 /Yr.
