Job Description

Summary:
Provides leadership to the health care team in areas of coordination of post-acute care services and provision of psychosocial support services to patients and families. Serves as a resource for the health care team on appropriate and timely utilization of acute care level services and length of stay. Assists with departmental operations and improvement projects. Responsibilities Conduct comprehensive psychosocial assessments to identify patients' emotional, social, cultural, spiritual, and care coordination needs, and develop collaborative treatment and discharge plans with interdisciplinary healthcare teams. Provide crisis intervention, counseling, and emotional support to patients and families while facilitating support groups and educational sessions for patients, caregivers, and community members. Coordinate patient admissions and ongoing case management activities, including client screening, transportation arrangements, authorization procurement, communication with funding sources, and management of denials and appeals. Participate in patient rounds, care conferences, and interdisciplinary meetings to support treatment planning, care modifications, and continuity of care across the healthcare continuum. Maintain accurate and timely documentation of assessments, interventions, treatment plans, and outcomes in compliance with hospital policies, regulatory standards, and reporting requirements, while escalating concerns to leadership as appropriate. Contribute to program development, performance improvement initiatives, and outcome analysis while fostering collaborative relationships with patients, families, colleagues, and community stakeholders through flexibility, teamwork, and leadership. Qualifications LMSW, LCSW or LCSW-C licensure endorsed by the state of Maryland Hospital experience preferred
