Pine Point Center, a Genesis HealthCare community, is seeking a compassionate Social Worker to join our Social Services team. In this role, you will support patients and families throughout short-term rehab and long-term care, conducting psychosocial assessments, discharge planning, and counseling. You will collaborate closely with an interdisciplinary clinical team to promote patient-centered care, advocacy, and smooth care transitions. Responsibilities Conduct comprehensive psychosocial assessments for patients in post-acute, rehabilitation, skilled nursing, and long-term care settings Develop and implement individualized psychosocial care plans in collaboration with the interdisciplinary team Coordinate and manage discharge planning, including evaluation of support systems and arrangement of community-based services Provide counseling, emotional support, and crisis intervention to patients and families coping with illness, loss, and changes in function or independence Facilitate safe and timely transitions of care to home, assisted living, long-term care, or other appropriate settings Participate in care conferences and interdisciplinary team meetings, clearly communicating psychosocial needs and plans Advocate for patient rights, preferences, and dignity, helping to resolve concerns and address barriers to care Maintain thorough and timely documentation of assessments, interventions, progress notes, and discharge plans in the medical record Educate patients and families about available community resources, financial assistance, and entitlement programs Ensure compliance with regulatory standards, facility policies, and professional social work ethics, including confidentiality and HIPAACollaborate with nursing, rehabilitation, and medical staff to support high-quality clinical outcomes and a therapeutic environment Contribute to quality improvement initiatives and participate in professional development activities as requested Required Skills Psychosocial assessment Discharge planning and care transitions Case management Individual and family counseling Crisis intervention Knowledge of community resources and social service systems Healthcare and long-term care regulations familiarity Patient advocacy Interdisciplinary team collaboration Documentation and electronic medical records (EMR) proficiency Care planning Conflict resolution and de-escalation Time management and organizational skills Cultural competence in patient and family interactions Knowledge of insurance, Medicare/Medicaid, and entitlement programs
