FULL TIME
Social Worker to be shared between sister facilities Adrian Bay & Springcreek. This position will support the Social Services Director with day to day functions without the skilled nursing facility. We're looking for a compassionate, organized, and resident-focused Social Worker to support two closely partnered buildings located under 2 miles apart . This role offers the best of both worlds: meaningful variety in your day while staying rooted in one company! About the Role As our Social Worker, you'll play a key role in supporting residents, families, and the interdisciplinary care team across both locations in Adrian, MI. The facilities collaborate closely, allowing for continuity, teamwork, and shared support. What You'll Do Provide psychosocial support to residents and families Assist with admissions, care planning, and discharge planning Coordinate with interdisciplinary teams to support resident needs Support residents through transitions and changes in care Ensure documentation meets regulatory and facility standards Serve as an advocate for residents' emotional and social well-being Why This Role Is Unique Full Time shared between two buildings, under 2 miles apart. 3 days a week in one facility and 2 days a week in the other, then flip the following week.
