Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Social Worker (LSW) to join our Maternal Infant Health team. The person will play a vital role in bridging the gap between health providers and the community. This position involves engaging with individuals and families to promote health education, assist with navigating healthcare systems and provide support various health-related needs. The LSW will work closely with diverse population to enhance access to Maternal Infant Health Program (MIHP), healthcare services, improve health outcomes and empower community members through education and resources. Duties Conduct thorough patient assessments to identify psychosocial needs, including mental health status, family dynamics, and community resources. Develop personalized care plans aligned with clinical ethical guidelines and social work practice standards. Conduct outreach activities to engage community members and promote health programs. Educated individuals and families about available MIHP and healthcare services, including Medicaid options. Utilize motivational interviewing techniques to assess client needs and develop personalized action plans Experience Proven experience as a Licensed Social Worker in healthcare environments with families. Demonstrated expertise in child & family counseling. Familiarity with medical terminology, ICD coding systems (ICD-9/10), and Medicaid regulations. Hands-on experience in crisis management, discharge planning, case management, and behavioral health interventions. Knowledge of working with individuals from diverse cultural backgrounds and those with disabilities or developmental disorders. Ability to collaborate effectively within clinical interprofessional teams while maintaining high standards of confidentiality and ethical practice. Pay:
$29.00 - $30.00 per hour
