  • 4:30pm plus weekend on-call rotation one weekend every 2 months

What You'll Do, includes but is not limited to: Conduct initial psychosocial assessments on prospective program participants, consult with family or other support system members Develop clear and timely written social service care plans and implement for assigned participants. Review social service care plans as needed or required and revise as appropriate. Assist families with problem solving around care giving issues and serve as the facilitator of family meetings as needed. Provide crisis intervention and advocacy as needed Provide supportive counseling to participants and families as needed

  • Friday, 8:00am
  • 4:30pm Participate in the weekend on-call rotation: one weekend every 2 months What We're Looking For Requires Master's Degree in Social Work and current state of Michigan Social Workers license LMSW (limited license LLMSW acceptable with expectation to earn full licensure in a timely manner) Prefer at least 2 years of relevant experience Minimum of 1 year experience working with the frail or elderly population Ability to manage multiple tasks comfortably while staying organized and professional Work well autonomously as well as within a team setting Ready to Apply?
