3002585355-2 Apply for this job now Job Description Thriveworks is seeking a Limited Master's Social Worker to provide high-quality outpatient mental health services to individuals, couples, and families. In this role, you will complete biopsychosocial assessments, develop collaborative treatment plans, and deliver evidence-based counseling in-person and via telehealth. You will coordinate care with psychiatrists and other providers, manage risk and crisis situations, and maintain accurate, timely documentation in our EHR. Responsibilities Provide outpatient individual, couple, and family counseling in-person and via telehealth Conduct comprehensive biopsychosocial assessments and formulate diagnoses as permitted by license Develop and implement collaborative, evidence-based treatment plans Maintain accurate, timely clinical documentation in the electronic health record Coordinate care with psychiatrists, primary care, and other providers as needed Perform risk assessments, safety planning, and crisis intervention when indicated Support clients in accessing community resources and social supports Participate in clinical consultation, supervision (if required), and team meetings Uphold ethical, legal, and documentation standards for clinical practice Contribute to an inclusive, client-centered, and outcomes-focused culture at Thriveworks Required Skills Clinical assessment and diagnosis Treatment planning Evidence-based psychotherapy Case management and care coordination Crisis intervention Electronic health record (EHR) documentation Insurance and billing documentation Risk assessment and safety planning Interdisciplinary collaboration Cultural competency
