The Individual Family Support Coordinator works closely with the manager and team member to provide Perinatal Mood and Anxiety Disorder education, resources and referrals to Corewell Health families in Southeast Michigan. The hours are variable as we offer evening Parent Connection Groups for first time parents to assist in their new parenthood roles. Coordination with team members is an essential part of this role. Within the first year of employment education in Perinatal Mood and Anxiety Disorders through Postpartum Support International is required, or equivalent.Scope of work:
The Individual Family Support Coordinator will support Beaumont Health patients identified as elevated risk for postpartum mood and anxiety disorders and will supply parent education and support to Beaumont families through virtual classes and support groups. The Individual Family Support Coordinator will recruit, train, and supervise qualified volunteers to aid in these endeavors. Will work with proper staff and community agencies to gather information and resources for individual families. Determine mothers at elevated risk for developing postpartum mood disorders through completed inpatient EPDS scores, referrals from Social Work, physician referrals, and self-referrals. Contact these patients to decide their needs and provide education, resources, and/or referrals. Document contacts and actions taken. The Individual Family Support Coordinator will conduct weekly virtual Postpartum Adjustment Support Groups and follow up with families as needed. Develop and conduct a virtual class for prenatal parents to educate about the major transition to parenthood and how to help facilitate this transition. Participants will be assisted in developing realistic expectations of parenthood, effective communication strategies with their partners, and coping skills. The class may include an overview of baby care basics, breastfeeding basics, and perinatal mood disorders. Relevant resources will be provided. Development, coordination, and supervision of weekly virtual Parent Connection Groups for parents who delivered at each Beaumont hospitals. These groups will include an educational component, as well as a socialization component, to facilitate the development of a support system for participating families. Inform health care providers of the education, support, and resources available to parents. Keep providers informed of resources and training available to them about perinatal mood disorders and their treatment. Communicate with providers to find other services that would benefit their patients. Act as a resource on parenting and perinatal mood disorders for patients, hospital staff, and healthcare providers. Attend conferences, webinars, and local educational opportunities to expand knowledge and skills and to keep up to date of current information about perinatal mood disorders and early parenthood. Qualifications Required Associate's Degree Completion of degree in social service related field Required Master's Degree MSW required Preferred Completion of training in perinatal mood disorders through Postpartum Support International, or an equivalent level is strongly preferred. Can be completed in the first year. Experience supervising volunteers; education or extensive knowledge in infant development, parenting issues, group dynamics, and perinatal mood disorders. Required Extensive work with supervising volunteers. Required Work in hospital and community setting.
