PRN Hospice Social Worker (MSW)

Coverage-based; hours are not guaranteed Position Summary Bayside Wellness Hospice is seeking a compassionate and dependable PRN Social Worker (MSW) to provide coverage when our regularly scheduled social worker is on vacation or otherwise unavailable. This is not a full-time position. Assignments will be offered on an as-needed basis and may include routine visits, psychosocial assessments, interdisciplinary team participation, and other hospice social work services. The Medical Social Worker provides emotional, psychosocial, and practical support to patients and families coping with terminal illness and end-of-life concerns. Working as part of the interdisciplinary team, this position helps patients and caregivers navigate emotional challenges, access community resources, and plan for ongoing care needs. Responsibilities Conduct comprehensive psychosocial assessments for assigned hospice patients and families. Develop individualized social work plans of care in collaboration with the interdisciplinary team and revise them as patient or family needs change. Provide counseling, crisis intervention, and emotional support related to terminal illness, grief, loss, and the dying process. Connect patients and families with appropriate financial, social, and community resources, including Medicaid, respite care, funeral planning, and other support services. Educate patients and caregivers about hospice philosophy, patient rights, and available community resources. Participate in interdisciplinary team meetings, case conferences, and care-planning discussions as needed. Complete accurate, timely, and thorough documentation in the electronic medical record. Maintain documentation and provide services in accordance with the patient's plan of care and applicable CHAP, CMS, state, HIPAA, and organizational requirements. Advocate for patient and family needs while maintaining professional boundaries and cultural sensitivity. Coordinate with nursing, bereavement, spiritual care, and other members of the hospice team. Follow all confidentiality, infection-control, safety, and quality-improvement policies. Required Qualifications Master of Social Work degree from a school accredited by the Council on Social Work Education. Current Michigan licensure as a Licensed Master Social Worker (LMSW) or Limited Licensed Master Social Worker (LLMSW), as applicable. At least one year of social work experience in a healthcare setting. Valid driver's license, current automobile insurance, and reliable transportation. Ability to travel throughout the Bayside Wellness Hospice service area for visits in private homes, assisted-living communities, and other care settings. Strong assessment, counseling, crisis-intervention, communication, and documentation skills. Ability to work independently while collaborating effectively with an interdisciplinary team. Preferred Qualifications Previous hospice, palliative care, home health, or end-of-life care experience. Familiarity with hospice Conditions of Participation and CHAP standards. Experience documenting in Netsmart or a similar electronic medical record system. Knowledge of community resources available to patients and families in the Traverse City area. PRN Compensation and Scheduling This is an as-needed position intended primarily to provide coverage when the regularly scheduled Medical Social Worker is unavailable. Hours, visits, and assignments are not guaranteed. Compensation may vary based on the type of service provided, including routine visits and psychosocial assessments, and will be discussed with applicants during the interview process. Required training will be paid at an hourly rate. Approved work-related mileage will be reimbursed. Physical and Work Requirements Ability to travel throughout the hospice service area. Benzie, Grand Traverse, Antrim, and Leelanau Counties. Ability to work in a variety of private home and healthcare facility environments. Ability to lift and carry supplies weighing up to 25 pounds. Ability to maintain strict confidentiality and comply with HIPAA requirements.
