3002585284-2 Apply for this job now Job Description Thriveworks is seeking a Limited Master's Social Worker to provide high-quality outpatient counseling to individuals, couples, and families. In this role, you'll complete biopsychosocial assessments, develop collaborative treatment plans, and deliver evidence-based therapy in person and via telehealth. You will document care in an EHR, coordinate with psychiatrists and other providers, and support ethical, inclusive, client-centered practice. Responsibilities Provide outpatient mental health counseling to individuals, couples, and families in person and via telehealth. Conduct comprehensive biopsychosocial assessments and formulate DSM-5-informed diagnoses. Develop and update collaborative, measurable treatment plans aligned with client goals. Deliver evidence-based therapeutic interventions and monitor client progress. Document sessions, assessments, and treatment plans accurately in the EHR and within required timeframes. Perform risk assessments, safety planning, and crisis intervention as needed. Coordinate care and consult with psychiatrists, primary care, and other providers. Maintain compliance with ethical, legal, and documentation standards, including insurers' requirements. Participate in supervision, case consultation, and professional development activities. Contribute to an inclusive, client-centered, and culturally responsive care environment.
