Job Description

Rightway Home Care Services, Inc is seeking to hire a Social Worker/QP who is willing to work with clients in the Twin Cities and surrounding suburbs.

JOB DESCRIPTION

Key Responsibilities:

Assist clients in locating and securing appropriate housing options in the community. Coordinate and collaborate with service providers, community resources, and families to develop individualized transition plans. Perform client services supervisory visits. Intake of clients Navigate and manage community and waiver programs, including CADI and Elderly Waiver. Complete individual and regular assessments of clients to assure their needs will be met. Support clients through discharge planning, housing coordination, and case management. Consistent communication with support team and completion of case documentation

PCA/CFSS

supervision Maintain accurate client records using Microsoft 365 and Microsoft Teams. Provide assistance during discharge days, including light lifting (up to 25 lbs.). Other duties as needed.

Qualifications:

Bachelor's Degree in Social Work, Psychology, Sociology, Human Services, Nursing, or a related field. At least two years of experience in human services, social services, or healthcare. Knowledge of community resources, waiver programs, and county applications. Experience with medical assistance, discharge planning, and housing coordination. Clearance to work in human services through a DHS background check. Reliable vehicle with valid driver's license and insurance. Proficiency with Microsoft 365 and Teams. Great communication skills 1. Must be licensed Social Worker in the State of Minnesota and must maintain licensure. 2. Current CPR certification and maintenance of certification preferred. 3. Has a valid driver's license with good driving record, must have reliable transportation.
