JOB FUNCTION
Provide flexible, family-centered coaching, advocacy, and support services across both Housing and Economic Empowerment programs, ensuring continuity of services. Partner with individuals and families to increase housing stability, economic security, and self-sufficiency while effectively managing competing priorities, maintaining clear communication, and coordinating resources both internally and within the community.
QUALIFICATIONS
Minimum Bachelor's degree in social work or human service-related field Valid driver's license and current vehicle insurance Preferred Experience working with adults with mental illness Minnesota Social Work License Lived experience or a history working with individuals with low incomes

ESSENTIAL FUNCTIONS
a) Provide family-centered coaching, case management, advocacy, and resource navigation services across both Housing and Economic Empowerment programs, supporting participants in achieving housing stability, employment, financial well-being, and self-sufficiency b) Conduct outreach, intake, assessment, eligibility determination, goal planning, and ongoing participant support while connecting individuals and families to CAPLP programs and community resources c) Maintain accurate and timely documentation of participant contacts, services, demographics, outcomes, and progress in required data collection systems. Complete program reports and other documentation requirements d) Serve as a cross-trained staff member by effectively managing competing priorities, maintaining accurate documentation, and communicating proactively with participants, team members, and community partners e) Build and maintain collaborative relationships with landlords, employers, service providers, and community organizations to ensure coordinated, high-quality services and positive participant outcomes Agency and Division Support a) Represent Agency in communities we serve b) Align and build on efforts to achieve a shared vision and common goals in serving people c) Assist with Agency projects as requested d) Attend and participate in agency, community and state meetings as requested e) Participate in agency provided trainings and seek out additional training to enhance professional development f) Assist with onboarding and training of Agency team members as requested g) Compile and submit required documentation h) Availability to work designated Agency hours for your Division and position Commit to, embrace, and uphold the CAPLP Core Values 1) Raise the
