Cura of Willmar is looking for a compassionate, approachable, and resident-centered Resident & Family Services Social Worker to join our team four days per week . As our Resident & Family Services Social Worker, you will serve as a trusted resource, helping residents and families navigate questions, concerns, and available services while collaborating with our interdisciplinary team to enhance the resident experience. Position Summary The Resident & Family Services Social Worker is responsible for providing psychosocial support to residents and serving as a primary resource for residents and their families. This position focuses on building positive relationships, addressing questions and concerns, assisting with care planning, advocating for resident needs, and helping ensure a positive experience throughout each resident's stay. This position is scheduled four days per week . Essential Responsibilities Resident & Family Support Build positive relationships with residents and their families. Serve as a resource for residents and families by answering questions and providing guidance regarding facility services and resident care. Meet with residents and families throughout their stay to ensure their needs are being addressed. Provide emotional support, counseling, and referrals to community resources as appropriate. Assist residents in maintaining the highest possible quality of life. Resident & Family Communication Address resident and family questions or concerns promptly, professionally, and compassionately. Collaborate with department leaders to help resolve concerns and improve the resident experience. Promote open communication between residents, families, and the interdisciplinary team. Follow up to ensure concerns have been addressed and residents and families feel supported. Care Planning & Advocacy Complete psychosocial assessments and required social service documentation. Participate in interdisciplinary care conferences and resident care planning meetings. Advocate for resident rights, preferences, dignity, and independence. Assist residents and families with advance directives and other social service needs as appropriate. Quality & Compliance Maintain accurate and timely documentation in accordance with state and federal regulations. Maintain resident confidentiality and comply with HIPAA requirements. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives. Assist with resident satisfaction initiatives and continuous quality improvement efforts. Team Collaboration Work closely with nursing, therapy, activities, dietary, admissions, environmental services, and administration. Help foster a welcoming, supportive, and resident-centered environment. Participate in department and interdisciplinary meetings as assigned. Requirements Bachelor's degree in Social Work preferred. Minnesota Social Work licensure preferred. Previous experience in long-term care, healthcare, case management, or customer service is preferred. Excellent communication, interpersonal, and conflict-resolution skills. Strong organizational and problem-solving abilities. Ability to work collaboratively within an interdisciplinary healthcare team.
