Job Description

This position offers a unique opportunity to work across multiple clinic locations, programs, and counties while helping individuals and families access behavioral health services and community resources. You will provide essential coverage for staffing needs, assist clients with care coordination needs, and ensure continuity of services across our organization. This role is ideal for someone who thrives in a fast-paced environment, enjoys variety, and is passionate about helping others. Full-time position, working Monday through Thursday 7:30am-5:00pm, and Friday 8:00am-12:00pm.
Job Duties:
Provide unit coverage across multiple Woodland Centers locations. Coordinate services among health care providers, schools, and community agencies. Provide care coordination services to clients receiving behavioral health services. Assist clients in overcoming barriers to care, including transportation, housing, financial, and community support needs. Monitor screenings, referrals, treatment progress, and follow up needs.
Requirements:
High School Diploma or equivalent. Experience working in behavioral health, human services, or related fields. Experience in care coordination, case management, or behavioral health services. Demonstrate strong, independent decision making and problem-solving skills. Ability to travel frequently throughout the Woodland Centers service area. Maintain a valid driver's license, reliable transportation, proof of insurance, and a clean driving record.
