A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.

Job Responsibilities and Duties:
Plan, develop, organize, implement, evaluate, and direct the overall operations of the Social Services Department Oversee the Medicaid process Complete MDS assessments and support the admissions process Provide leadership and guidance to the Social Services Department staff Work primarily during daytime hours, with occasional evening and weekend visits as needed
Requirements:
Bachelor's degree from an accredited school or program of Social Work (minimum requirement) Two years of postgraduate experience in long-term care or related programs Must meet equivalent state requirements for social work practice, or hold certification/credentialing from the National Association of Social Workers if the state has no legal regulation BS in Social Service required 2+ years of social work and MDS assessment experience, ideally in a long-term care setting Energetic, results-oriented team leader with superior interpersonal and communication skills Self-motivated, caring approach, with passion for working with elderly and behavioral residents
