Healthy Start Case Manager Job Type:
Full-Time Location:
Sikeston, MO (On-site only- No Remote)
Schedule:
Monday to Friday, 8:00 AM- 5:00 PM (40 hours/week)
Salary Range:
Mid-high $30,000s- low-mid 40; s (Negotiable based on education and experience) Position Summary Provides comprehensive care coordination, advocacy, and education to pregnant and postpartum women and their infants (up to 18 months old).
Service Region:
Butler, Dunklin, Mississippi, New Madrid, Pemiscot, and Scott Counties Key Responsibilities- Manage an assigned caseload effectively.
- Demonstrate strong interpersonal skills and a commitment to providing excellent customer service.
- Conduct telehealth visits to assess family needs and deliver education on Maternal Health topics.
Education/Certifications:
Bachelor's or associate Managementdegree in social work, Public Health, or a related field preferred. Community Health Worker (CHW). Some college coursework preferred; relevant experience may substitute for education requirements. Experience (Preferred): Minimum of 1-2 years in case management, maternal-child health, public health, or a related field. Knowledge of the Service Region's geography and demographics. Experience working with pregnant women, infants, and families. Ability to manage an assigned caseload effectively. Proficiency in web-based software, data entry, and Microsoft Office applications.Physical Requirements:
Ability to sit for extended periods and lift objects up to 35 lbs. How toApply:
