Social Worker Case Manager Duties:

Work as a two-person team (registered nurse & licensed social worker) to develop and initiate an appropriate plan of services and support for qualified individuals. Make in-home, face-to-face monthly visits with clients in company provided vehicle. Monitor, review, and make revisions, where necessary, to clients' service plan. Coordinate the efforts of family, friends, and/or volunteers to provide services to clients. Knowledge and skill in clinical assessment techniques, understanding of geriatrics and disabilities beneficial. Ability to work in a team relationship required. Must be licensed to work in Mississippi and pass a criminal background check.
