Job Summary:

Under the supervision of a designated program manager or supervisor where applicable, performs home visiting parent education and social work services in the community and in client homes specific to grant requirements. Essential Functions (Major Duties or Responsibilities): These duties are the essential functions and are not all-inclusive of all duties that the incumbent performs. Provide structured education, support, and resource referrals to clients and families assigned to caseload. Perform standard evaluations and assessments related to client/family wellbeing. Facilitate individual client and family goals based on social and developmental needs. Monitor and evaluate goal progress. Document home visits with clients/families according to requirements and assist in reporting of data. Documentation may infrequently be used in court settings. Triage socially related needs of clients. Coordinate client referrals utilizing appropriate community resources. Identify available community resources for clients, families, and populations served and act as a social service resource person for the agency. Educate clients on the appropriate use of social services. Perform outreach to client populations and social services organizations to increase referrals and community reach of program. Identify and make contact with high risk clients and specific target populations. Collaborate with other programs within Division and with community partners serving mutual clients and/or shared target population. Participate as the department representative on a professional or community association. Attend program-related meetings and conferences, which may require overnight stays.

Physical Demands and Working Conditions:

Work is performed in office, client home, and community environments. Work requires the ability to lift and carry 40 pounds and be able to navigate stairs. Work requires frequent operation of a Flathead County vehicle. Must possess current Montana Driver's License and clear driving record.

Supervision Exercised:

This is a non-supervisory position.

Knowledge, Skills, and Abilities:

Extensive knowledge of community resources and organizations. Knowledge of the practices and procedures of the professional field of social work. Knowledge of the psycho-social aspects of public health disciplines, particularly related to child development and family dynamics Skill in interviewing techniques, listening, facilitating problem solving, and crisis intervention. Skill in promoting grant-based program and securing referrals. Ability to understand and carry out oral and written instructions of a professional nature. Ability to communicate well verbally and in writing. Ability to work effectively as a member of an interdisciplinary team. Ability to establish and maintain effective working relationships with the public, staff, and other professional people contacted in the course of work.

Education and Experience:

Requires a bachelor's of art or science degree in an applicable course of study and one year of experience in field related to child development, social work, or community health education; or equivalent professional level work experience; or any combination of training and experience which indicates possession of the knowledge, skills, and abilities listed. One year of relevant professional-level work experience may be substituted for one year of required education given that it is in the same field.
