As a Service Coordinator, you will play a vital role in helping residents access the resources, services, and support they need to age successfully in place and enjoy a high quality of life.

What You'll DoAs the Service Coordinator, you will serve as a trusted advocate and resource for residents living in Matthew's Place HUD 202 Senior Housing community.
Key responsibilities include:
Conduct assessments to identify residents' social, health, and support needs.
Provide case management, resource referrals, and service coordination.
Connect residents with community services such as transportation, meal programs, counseling, wellness programs, legal resources, personal assistance, and healthcare services.
Build and maintain strong relationships with local agencies, service providers, and community partners.
Advocate for residents and help them navigate applications, services, and available benefits.
Develop individualized service plans and monitor ongoing support services.
Help residents strengthen relationships with family, friends, neighbors, and community resources.
Coordinate educational programs and training that support successful aging in place.
Recruit and develop volunteer opportunities that enhance resident engagement and well-being.
Partner with staff to promote a resident-centered environment that values dignity, independence, and quality of life.

Is highly organized and able to manage multiple priorities.
Demonstrates empathy, professionalism, and sound judgment.

Is passionate about helping people access the resources they need to live independently.
Qualifications RequiredBachelor's degree in Social Work or a related field ORMinimum of five (5) years of experience in social services.
PreferredAt least one (1) year of experience working with older adults or in a geriatric setting.
