Job Summary and Qualifications The Discharge Panner coordinates patients' transition from the facility to next level care, ensuring a safe and timely discharge. They assess patient needs, collaborate with physicians, nurses, families and community providers, arrange post-discharge services, verify insurance coverage, and educate patients on their discharge plan to support continuity of care and reduce hospital readmissions.
